Outcome: distinguish a documented 72-hour review from a clinical outcome. Case: A fictional ward reports an increase in documented review after a new process. Discuss: What else might explain the change? Suggested answer: documentation practice, case mix or concurrent service changes could have changed; a before–after comparison cannot isolate the cause.
